What are the best ways to get rid off Cholesterol, High Blood Pressure, and Diabetes?

I am 52 Years old male and I have High Blood Pressure and my level is 140s *top one* and 90s *bottom one* problem, High Cholesterol and my Diabetes is in 140s.
